Urologist shot dead in exam room; man in custody

(AP) ? A 75-year-old retired barber was jailed Tuesday for investigation of killing a urologist in his exam room, but police have not yet released a motive.

Stanwood Fred Elkus of Lake Elsinore was arrested following the Monday afternoon attack and held on $1 million bail, according to the county jail website.

Elkus is suspected of killing Dr. Ronald Franklin Gilbert, 52, of Huntington Beach at the medical office in Orange County.

Police said they found Elkus and Gilbert in a second-floor room after several 911 callers reported six or seven shots fired. A gun was found at the scene.

The doctor had been shot several times in the upper body and was declared dead at the scene in Newport Beach.

Elkus had surgery for a prostate problem but he never specifically said Gilbert was his doctor, said his neighbor James Lord.

"He never complained to me about the doctor," Lord said.

On Sunday, Elkus asked Lord to fix the brake pedal on his car and "he kind of mentioned that he wasn't going to be around much longer," Lord said.

"I told him 'no Stan, you're gonna outlive me,'" Lord said.

Elkus, who lived alone, was familiar with guns, Lord said.

"We talked about guns before," he said. "He'd go out shooting, target practice. He'd go to target ranges.

"I just can't believe that he did it," Lord said. "He was a nice guy. ... He didn't seem like he was going to flip out or anything."

An autopsy was planned Tuesday and Elkus was scheduled to appear Wednesday in court. Police searched his home but declined to disclose what they found.

Gilbert, a married father of two children, appeared to be the only target of the attack, police spokeswoman Kathy Lowe said.

Dr. Jeffrey Lauber, a dermatologist, said he frequently referred patients with cancer symptoms to Gilbert.

"He's the best there is," Lauber said. "Not only is he a very good urologist, he's a pillar of the community."

Lauber couldn't imagine why anyone would want to shoot Gilbert.

"This doesn't happen down here," he said of the wealthy enclave 45 miles southeast of Los Angeles.

Gilbert dealt with general urology, sexual dysfunction and related surgical techniques including vasectomies, bladder and prostate cancer, according to his biography on the website of the Orange Coast Urology Group, which he joined in 1993.

One of his specialties involved using a laser to vaporize prostate tissue blocking the urinary tract. His biography called him a "people person" and said he enjoys helping people.

He decided to become a doctor mainly because his late father was a doctor, the biography said, adding that Gilbert had been a stockbroker and a singer in a rock band.

Feedback given on change of location of MLK Day parade

Hollis Briggs Jr. went first. ?Business access,? he said, ?it can be worked on. Improved.?

Sarah Gilliam was next. ?I marched the parade and I noticed that from ? it?s hard remember exactly without landmarks ? but from about Walnut to the finish, groups got really spaced out.?

After Gilliam came another woman, then a man, and around the table they went offering input on Wilmington?s Martin Luther King Day parade, which for the first time this year was held downtown instead of at its longtime location on Castle Street.

Briggs and Gilliam were among about 80 business owners, employees, parade participants and just interested persons who turned out for a debriefing session held Tuesday at Wilmington City Hall to collect feedback on the location change and ideas for improving the parade in years to come. Attendees split into eight or nine groups and, one at a time, voiced their opinions, which were then written down in marker by a city staff person on a flipboard placed on an easel, one for each table.

Concerns ran the gamut, from how to ameliorate effects on businesses to providing more seating for spectators. Viewpoints were also divided, offering a microcosm of public opinion.

The ideas put forward on Tuesday evening could influence not just how the MLK parade unfolds in the future, but how the increasing number of downtown events, such as the Azalea Festival, operate.

Wilmington is currently exploring ways to improve planning and processes for holding downtown events, the first such evaluation in nearly a decade, city officials said. Events such as races, festivals and parades are increasingly using downtown as a launching pad, and the city wants a framework to ensure that it can properly adapt and sustain that growth.

?When you look at having more events downtown, we have to look at revising our procedures and finding a compromise between the business interests and the event organizers,? said city spokeswoman Malissa Talbert.

The MLK Day parade moved downtown this year as organizers sought to attract a bigger crowd. It worked ? some estimates put the total at more than 12,000 attendees, a marked increase over years prior.

While it delivered the spectacle to a larger audience, the move also caused financial pain for businesses. Owners complained that road closures blocked customers? access to their restaurants and stores, a backlash that prompted Wilmington to host Tuesday?s meeting.

The city does not organize the MLK Day parade, but it issues permits for street closures.

Several groups were represented, including restaurants like Front Street Brewery, stores from the Cotton Exchange and downtown development organizations such as Wilmington Downtown Inc. Members of the city council and Mayor Bill Saffo also were present.

Opinions diverged. Some thought the move downtown was a positive change; others thought it deprived the Castle Street community and hurt black-owned businesses. Some saw parking as adequate; critics expressed frustration. Many thought the downtown businesses should have received better advance notice, but at least one person thought they were satisfactorily notified and should have voiced their concerns beforehand. Some criticisms carried racial undertones. A few asked why the MLK Day parade ignited such a kickback when other events like Azalea Festival did not.

?This has been great to get this communication going,? said Joan Loch, who co-owns Crescent Moon and chairs the group Bring It Downtown.

The president won't put forward a bill - instead he'll support the Senate's principles outlined yesterday and explain what else needs to be done. The White House feels Las Vegas is a community symbolic of the growing Latino population in both the state and the nation and since immigration reform is a pledge the president made during the campaign, the White House says he wants to deliver.

EL DIABLO IS IN THE DETAILS: Even the senators who wrote the immigration reform proposal outlined yesterday admitted there's lots of work still ahead. One land mine: Some Republicans want to link getting green cards to whether the border is secure. Border security still a gray issue. If the Gang of Eight's efforts fall apart the president's team will step in with its own proposal, Ninan notes.

SECRET CONGRESSIONAL GROUP WORKING ON IMMIGRATION ALTERNATIVE: A separate bipartisan group of lawmakers in the House of Representatives is on the verge of finalizing its own designs for comprehensive immigration reform, ABC's John Parkinson reports. The discussions, which top aides close to the talks discussed on the condition that they not be identified, are described as "Washington's best-kept secret." Multiple sources say those involved in the talks include Democratic Reps. Xavier Becerra (California), Luis Gutierrez (Illinois), Zoe Lofgren (California), and Republican Reps. Mario Diaz-Balart (Florida), Sam Johnson (Texas) and John Carter (Texas). The House's not-yet-finalized proposal is expected to address five general areas of immigration reform, according to aides close to the negotiations. Secure the border, implement a permanent E-verify system nationwide, reform the visa system, address the predicament of how to handle immigrants already in the country illegally in a "fair" and "legal manner" while determining how to handle those who have applied for legal immigration and are currently waiting in line, and reform the immigration system for future applicants. http://abcn.ws/WnjOfh

ABC's RICK KLEIN: What could possibly go wrong? The bipartisan Senate proposal is on the table, with boldfaced names like McCain, Rubio, Graham, Schumer, Durbin, and Menendez signed on. The House isn't far behind. And the president takes up the mantle himself today, as he lobbies the public to force action at last on immigration reform ? Wait, this could get interesting, after all. The White House has had mixed results with letting Congress handle the details of much of anything. But these are the kinds of details that members of Congress from both parties have spent months if not years wrestling through; witness the twin failures of immigration reform, in 2006 and 2007, under the leadership of a different president. The real question for the White House: Will heavy involvement - and pushing in directions the Gang of Eight doesn't want to go - be more harmful for helpful?

ABC's MICHAEL FALCONE: At yesterday's bi-partisan news conference announcing the Gang of Eight's immigration reform principles, Sen. John McCain's answer about why Republicans were so eager to move on the issue was telling. "Elections, elections," the Arizona senator said. "The Republican Party is losing the support of our Hispanic citizens." He's right: Hispanic voters are becoming a larger share of the electorate and GOP presidential candidate, Mitt Romney, won just 27 percent of the vote among the group compared to 71 percent who supported President Obama. There was also something striking about watching Florida Sen. Marco Rubio, a member of the immigration reform group, launch into Spanish during the press conference. Neither Rubio's language skills nor his familiarity with the immigration issue are breaking news, but I imagine it will give other potential 2016 Republicans pause.

ABC's SHUSHANNAH WALSHE: Currently, the prevailing theory about Sarah Palin is that because she doesn't have the megaphone of Fox News anymore, the "Palin moment" is now officially over. It might be true, but there have been so many "Ends of Sarah Palin" that it's almost hard to keep track. She was over when she lost the 2008 campaign, she was over when she quit the Alaska governorship, she was over when she decided to do a reality show, she was over when she decided not to run for president. Now she's over because she severed her ties with Fox. But the reality is different. Even after she decided to resign as governor and to pass up a 2012 presidential bid, people who both love her and hate her still just couldn't get enough information about her. Palin still got an incredible amount of coverage and her voice was heard - loud and clear. It's yet another example of what she's able to pull off that others who came before or after just aren't: She's been written off since Day One, but she keeps coming back.

ABC's JASON RYAN: The FBI has released new gun background check data yesterday showing that the week after the Newtown massacre (December 14, 2012) was the busiest for gun background checks ever, followed by the week President Obama announced new gun control proposals on January 16, 2013. As ABC News has reported, gun sales have been booming since Newtown. After previously denying journalists access to gun data, National Instant Check System figures show that overall in December 2012 there were more than 2.78 million background checks carried out to purchase firearms surpassing the previous record from November 2012 when more than 2 million checks were performed. The number of total sales during the first month of the new year will be released in the first few days of February.

VIDEO OF THE DAY: MEET DEFIANT DEMOCRAT, HEIDI HEITKAMP. Sen. Heidi Heitkamp, D-N.D., defied the odds in November when she won the closest senate race in the country, and now that she's arrived in Washington, she's defiant as ever. But now, instead of defying the pollsters, she's defying the Democratic caucus by taking divergent opinions on issues central to the President Obama's second term agenda, ranging from gun control to the environment. Heitkamp, who says growing the economy is her top priority, is concerned that the president is changing his focus to issues like climate change and gun control. "I think, you know the one thing that has gotten lost by everyone is one of the best ways that we can perform here is by getting people back to work, making sure that this economic recovery, slow as it is, gets amped up and moves forward," Heitkamp tells ABC's Jonathan Karl, host of "Politics Confidential." IMMIGRATION REFORM PLAN INCLUDES A PATHWAY TO CITIZENSHIP. The Senate's plan does not grant undocumented immigrants automatic "amnesty," rather it requires them to go through an arduous process that includes undergoing a background check, paying fines, back taxes and learning English and American civics over the course of a number of years, reports ABC-Univision's Jordan Fabian. The new law would grant eligible undocumented immigrants permission to live and work in the U.S. legally, but would not confer permanent legal status, or a green card, until the border is deemed to be secure. Young people brought into the U.S. illegally as minors and some agricultural workers would face an easier path to citizenship. "We will never put these people on a path to citizenship until we have secured the border," New York Sen. Chuck Schumer said yesterday. Sen. John McCain, R-Ariz., who helped lead the last effort on a comprehensive immigration bill in 2007 said, "We have been too content for too long to allow individuals to mow our lawns, grow our food, clean our homes, and even watch our children while not affording them any of the benefits that make our country so great." http://abcn.ws/YBGY4Q

OBAMA TALKS GUN VIOLENCE WITH POLICE CHIEFS. President Obama is enlisting the help of police chiefs from communities devastated by mass shootings as he continues a public push for Congress to act on his proposals to curb gun violence, ABC's Mary Bruce notes. "No group is more important for us to listen to than our law enforcement officials," the president told reporters before a White House meeting yesterday with sheriffs and police chiefs from across the country. "They are where the rubber hits the road." The president and members of his cabinet met with the police chiefs who responded to the deadly shootings in Aurora, Colo., Oak Creek, Wis., and Newtown, Conn, along with representatives from the Major Cities Chiefs Police Association and the Major County Sheriffs' Association. "I welcome this opportunity to work with them; to hear their views in terms of what will make the biggest difference to prevent something like Newtown or Oak Creek from happening again," Obama said.

CHICK-FIL-A CEO AND GAY ACTIVIST FIND COMMON GROUND. The leader of a national gay-rights group says he's coming out-as a friend of Chick-fil-A CEO Dan Cathy, ABC's Chris Good reports. "I've gotten to know Dan, he's gotten to know me. He's shared concerns about young people, about Chick-fil-A being used for certain purposes," Shane Windmeyer, executive director of Campus Pride, told ABC News. Last year, Cathy sparked a national controversy by telling a radio host that "we're inviting God's judgment on our nation when we shake our fist at him and say we know better than you as to what constitutes a marriage. And I pray God's mercy on our generation that has such a prideful, arrogant attitude that thinks we have the audacity to redefine what marriage is all about." Windmeyer said that Cathy called him last year, during the heat of the controversy that led national gay-rights groups to protest Chick-fil-A. Cathy reached out seeking advice and understanding, Windmeyer said. Windmeyer was a guest of Cathy's at this year's Chick-fil-A Bowl between LSU and Clemson at the Georgia Dome in Atlanta. Amazon posts $21.27 billion in 2012 Q4 revenues, makes tinier profit of $97 million

It's still the same old story, really. Amazon pulls in revenues equal to a small nation's GDP and makes a small profit for itself in the process. In this case, particularly small. Jeff Bezos' little company that could has posted fourth-quarter turnover of $21.27 billion, a 22 percent increase, but a net profit of $97 million that's down 45 percent from a year earlier. The discrepancy is attributed to a shift in reading habits that Bezos claims the company was expecting. Kindle downloads have been growing quickly, but old-fashioned paper book sales grew just 5 percent in December -- the lowest in company history, Bezos says. That's not surprising when you look at Amazon's bestseller charts, as the Kindle Fire HD and three other Kindle devices led the ranks. Most of the concern centers on the future. Amazon expects its operating income to dip from $405 million to anywhere between a $285 million loss and a $65 million profit, which doesn't exactly set a high bar for post-holiday performance.

Amazon.com Announces Fourth Quarter Sales up 22% to $21.27 Billion
Operating income up 56% year-over-year - above high end of guidance

SEATTLE, Jan 29, 2013 (BUSINESS WIRE) -- Amazon.com, Inc. AMZN +11.16% today announced financial results for its fourth quarter ended December 31, 2012.

Operating cash flow increased 7% to $4.18 billion for the trailing twelve months, compared with $3.90 billion for the trailing twelve months ended December 31, 2011. Free cash flow decreased 81% to $395 million for the trailing twelve months, compared with $2.09 billion for the trailing twelve months ended December 31, 2011. Free cash flow for the trailing twelve months ended December 31, 2012 includes fourth quarter cash outflows for purchases of corporate office space and property in Seattle, Washington, of $1.4 billion.

Common shares outstanding plus shares underlying stock-based awards totaled 470 million on December 31, 2012, compared with 468 million one year ago.

Net sales increased 22% to $21.27 billion in the fourth quarter, compared with $17.43 billion in fourth quarter 2011. Excluding the $178 million unfavorable impact from year-over-year changes in foreign exchange rates throughout the quarter, net sales grew 23% compared with fourth quarter 2011.

Operating income increased 56% to $405 million in the fourth quarter, compared with $260 million in fourth quarter 2011. The favorable impact from year-over-year changes in foreign exchange rates throughout the quarter on operating income was $2 million.

Net income decreased 45% to $97 million in the fourth quarter, or $0.21 per diluted share, compared with $177 million, or $0.38 per diluted share, in fourth quarter 2011.

"We're now seeing the transition we've been expecting," said Jeff Bezos, founder and CEO of Amazon.com. "After 5 years, eBooks is a multi-billion dollar category for us and growing fast - up approximately 70% last year. In contrast, our physical book sales experienced the lowest December growth rate in our 17 years as a book seller, up just 5%. We're excited and very grateful to our customers for their response to Kindle and our ever expanding ecosystem and selection."

Full Year 2012

Net sales increased 27% to $61.09 billion, compared with $48.08 billion in 2011. Excluding the $854 million unfavorable impact from year-over-year changes in foreign exchange rates throughout the year, net sales grew 29% compared with 2011.

Operating income decreased 22% to $676 million, compared with $862 million in 2011. The unfavorable impact from year-over-year changes in foreign exchange rates throughout the year on operating income was $14 million.

Net loss was $39 million, or $0.09 per diluted share, compared with net income of $631 million, or $1.37 per diluted share, in 2011.

-- For the second year in a row, Amazon's tablet was the most popular item for customers - Kindle Fire HD continued its run as the #1 best-selling, most gifted, and most wished for product across the millions of items available on Amazon worldwide. At year-end, Kindle Fire HD, Kindle Fire, Kindle Paperwhite and Kindle held the top four spots on the Amazon worldwide best seller charts since launch.

-- Amazon announced the launch of AutoRip, a new service that gives customers free MP3 versions of CDs they purchase from Amazon. Additionally, customers who have purchased AutoRip CDs at any time since Amazon first opened its Music Store in 1998 will find MP3 versions of those albums in their Cloud Player libraries - also automatically and for free.

-- Amazon introduced Kindle FreeTime Unlimited, bringing together for the first time all of the types of content kids and parents love - books, games, educational apps, movies and TV shows - into one simple, unlimited, easy-to-use service for kids ages 3-8.

-- Amazon's digital media selection has grown to over 23 million movies, TV shows, songs, magazines, books, audiobooks, and popular apps and games in 2012, an increase from 19 million at year-end 2011.

-- Amazon.com announced new licensing agreements with Turner Broadcasting, Warner Bros. Domestic Television Distribution, and A+E Networks, for popular television series including Falling Skies, The Closer, Pawn Stars, Storage Wars, and Dance Moms, expanding its catalog of title offerings for Prime Instant Video to more than 36,000 movies and television episodes.

-- Amazon launched Kindle Stores for Brazil, Canada, China, and Japan, with a large selection of the most popular books, including thousands of local-language books.

-- Amazon announced that 23 KDP authors each sold over 250,000 copies of their books in 2012, and that over 500 KDP Select books have reached the top 100 Kindle best seller lists around the world.

-- Amazon announced that for the eighth consecutive year, the company ranks #1 in customer satisfaction during the holiday shopping season according to the ForeSee annual Holiday E-Retail Satisfaction Index. ForeSee surveyed over 24,000 customers between Thanksgiving and Christmas, asking them to rate their satisfaction with the top 100 retailers. For the second year in a row, Amazon's score of 88 is the highest ever attained by any retailer in the study.

-- Amazon Web Services (AWS) announced the launch of its newest Asia Pacific Region in Sydney, Australia, now available for multiple services including Amazon Elastic Compute Cloud (EC2), Amazon Simple Storage Service (S3), and Amazon Relational Database Service (RDS). Sydney joins Singapore and Tokyo as the third Region in Asia Pacific and the ninth Region worldwide.

Patients' own skin cells are transformed into heart cells to create 'disease in a dish'

Jan. 27, 2013 ? Most patients with an inherited heart condition known as arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) don't know they have a problem until they're in their early 20s. The lack of symptoms at younger ages makes it very difficult for researchers to study how ARVD/C evolves or to develop treatments. A new stem cell-based technology created by 2012 Nobel Prize winner Shinya Yamanaka, M.D., Ph.D., helps solve this problem. With this technology, researchers can generate heart muscle cells from a patient's own skin cells. However, these newly made heart cells are mostly immature. That raises questions about whether or not they can be used to mimic a disease that occurs in adulthood.

In a paper published January 27 in Nature, researchers at Sanford-Burnham Medical Research Institute and Johns Hopkins University unveil the first maturation-based "disease in a dish" model for ARVD/C. The model was created using Yamanaka's technology and a new method to mimic maturity by making the cells' metabolism more like that in adult hearts. For that reason, this model is likely more relevant to human ARVD/C than other models and therefore better suited for studying the disease and testing new treatments.

"It's tough to demonstrate that a disease-in-a-dish model is clinically relevant for an adult-onset disease. But we made a key finding here -- we can recapitulate the defects in this disease only when we induce adult-like metabolism. This is an important breakthrough considering that ARVD/C symptoms usually don't arise until young adulthood. Yet the stem cells we're working with are embryonic in nature," said Huei-Sheng Vincent Chen, M.D., Ph.D., associate professor at Sanford-Burnham and senior author of the study.

To establish this model, Chen teamed up with expert ARVD/C cardiologists Daniel Judge, M.D., Joseph Marine, M.D., and Hugh Calkins, M.D., at Johns Hopkins University. Johns Hopkins is home to one of the largest ARVD/C patient registries in the world.

"There is currently no treatment to prevent progression of ARVD/C, a rare disorder that preferentially affects athletes. With this new model, we hope we are now on a path to develop better therapies for this life-threatening disease," said Judge, associate professor and medical director of the Center for Inherited Heart Disease at the Johns Hopkins University School of Medicine.

Disease in a dish

To recreate a person's own unique ARVD/C in the lab, the team first obtained skin samples from ARVD/C patients with certain mutations believed to be involved in the disease. Next they performed Yamanaka's technique: adding a few molecules that dial back the developmental clock on these adult skin cells, producing embryonic-like induced pluripotent stem cells (iPSCs). The researchers then coaxed the iPSCs into producing an unlimited supply of patient-specific heart muscle cells. These heart cells were largely embryonic in nature, but carried along the original patient's genetic mutations.

However, for nearly a year, no matter what they tried, the team couldn't get their ARVD/C heart muscle cells to show any signs of the disease. Without actual signs of adult-onset ARVD/C, these young, patient-specific heart muscle cells were no use for studying the disease or testing new therapeutic drugs.

Speeding up time

Eventually, the team experienced the big "aha!" moment they'd been looking for. They discovered that metabolic maturity is the key to inducing signs of ARVD/C, an adult disease, in their embryonic-like cells. Human fetal heart muscle cells use glucose (sugar) as their primary source of energy. In contrast, adult heart muscle cells prefer using fat for energy production. So Chen's team applied several cocktails to trigger this shift to adult metabolism in their model.

After more trial and error, they discovered that metabolic malfunction is at the core of ARVD/C disease. Moreover, Chen's team tracked down the final piece of puzzle to make patient-specific heart muscle cells behave like sick ARVD/C hearts: the abnormal over-activation of a protein called PPAR?. Scientists previously attributed ARVD/C to a problem in weakened connections between heart muscle cells, which occur only in half of the ARVD/C patients. With the newly established model, they not only replicated this adult-onset disease in a dish, but also presented new potential drug targets for treating ARVD/C.

What's next?

Chen's team was recently awarded a new grant from the California Institute for Regenerative Medicine to create additional iPSC-based ARVD/C models. With more ARVD/C models, they will determine whether or not all (or at least most) patients develop the disease via the same metabolic defects discovered in this current study.

Together with the Johns Hopkins team, Chen also hopes to conduct preclinical studies to find a new therapy for this deadly heart condition.

Endangered voles set to return

An endangered species is set to return to a Northumberland forest more than 30 years after it was wiped out.

Conservationists at Kielder Water & Forest Park plan to reintroduce water voles to the area after they disappeared.

The animals have not been spotted in the area since the 1970s.

Forestry Commission ecologist Tom Dearnley said they were "extremely keen" to see the project go ahead at Kielder.

Mr Dearnley said: "Areas like Kielder Burn and the North Tyne are good water vole habitats so we have a two-part plan which will hopefully see them return to former haunts.

"First we need to establish whether any mink remain as this was the reason for their previous decline.

"Water voles have suffered big declines across England, so returning them to the forest is something we are extremely keen to see happen."

Mink numbers at Kielder are now thought to be very low with few being spotted by rangers in recent years.

Steve Lowe, from the Northumberland Wildlife Trust, said a similar project had been carried out in the Cairngorms, which saw the water vole making an "impressive comeback".

?I?ve said for the past three years that I believe the tablet market will be larger than the PC market at some point,? said Cook, echoing Steve Jobs? assertion that some day?PCs will be like trucks: Only some people will need them, and only for heavy-duty tasks.

?Our base philosophy is to never fear cannibalization,? Cook added. ?If we do, somebody else will just cannibalize it.?

The overall PC market is much larger than the market for either Macs or iPads, so if Cook is right, there is tremendous opportunity for Apple to grow both market share and revenue as consumers ditch PCs for tablets. But that also means that Apple?s real competition is no longer other PC manufacturers but makers of tablets, particularly Samsung?and Google.

Let us first say that, as of writing this article, we have 126 Twitter followers. Now, don?t get us wrong, we are really proud of this number, but by no means does this make us experts. In fact, we are still figuring out how to get the most out of Twitter as we possibly can. We consider ourselves newbies to the Tweet scene, and are only beginning to understand its implications for education. But, in this short time, one thing has been made very clear: Twitter is definitely the current ?it??place for the most cutting-edge ideas in the education field.

At the teaching fellowship program we attended, we were told to make sure to visit another classroom teacher and observe because, ?classroom teaching can be a very solitary profession.? Perhaps it was prior to now, but with the explosion of social media, web-based professional development communities, and online learning management systems there is no need for an opaque and solitary teaching existence. Teaching can now be, thanks to the Internet, a transparent and shared venture. In the past, teachers could hoard good lessons and live in the solipsistic world of the classroom all day; now, teachers can share their progress, their ideas, their lessons, their struggles, and most importantly, their successes.

One of the most significant (and rightly so) hurdles of this sharing on the Twitterverse is that it is so so very public. When teachers use online learning management systems or class websites they are usually only opening themselves up to the students, parents, teachers, and administrators at their school. But posting lessons, outlines, experiences, and opinions on Twitter is for the world to consume. This has implications for the teacher socially and professionally. Schools may prefer that their teachers not engage in social media as public as Twitter in the fear that the sharing may reflect poorly on the school, or, at the very least, wish that their teachers? opinions be accompanied by the oft seen caveat of: ?the opinions expressed here do not necessarily reflect those of the school.?

To indulge in these fears completely would be to miss a groundbreaking professional development opportunity. ?A recent blog post by educator Tony Sinanis recapping his year in Twitter waxed eloquent on the myriad of reasons why Twitter is so amazing, even asking the tongue-in-cheek question: ?Do you want to watch an episode of Modern Family and participate in some PD at the same time? Twitter lets you do that!? (PD being the Twitter slang for professional development.) However witty Sinanis is being, he is completely correct. What Twitter offers is a twenty-four-hour, any subject/specialty course in professional development that you can do while living your personal and professional life. ?Administrators afraid of the publicness of Twitter need to see the flip side: because it?s so public, it is free, and allows a world of educators to share what is good and what is coming.

Educators are educated too. The two of us have favorited so many articles and blog posts with both theoretical and abstract education concepts, as well as straight-to-classroom use-this-minute ideas, that we have trouble keeping up. And while we are just getting the hang of moving from consumer to contributor in the incredible #edchat #edtech conversations, the amount of support, information, and cutting-edge practice out there is astounding.

It?s easy for a teacher to feel like the king of the classroom when you are there with your students, to feel like the education you received was sufficient, that the professional development conferences you attend keep you cutting edge enough? but until you look at Twitter, you won?t realize the sheer volume of conversations that are happening without you. This was what the two of us experienced when we joined Twitter. It was kind of scary and awe-inspiring at the same time: the education world had been going on without us, and it was going really fast.

Just in January alone, Twitter educators have discussed textbook reform, classroom and school data analysis, and the engagement of parents in schools. Articles and blog posts were written on cutting edge education topics: flipped classrooms, iPad use, digital citizenship, project based learning, differentiation, and more. Teachers have their pick of topics that apply to their experience and their students? needs, and have access to a vast resource of educators who have tried new approaches to teaching and can tell their stories. And these educators come from every place in the education sphere; there are principals, heads of school, classroom teachers, resource teachers, etc. ?from all types of schools, public and private?all sharing their knowledge together.

Twitter and other social media can?t (and shouldn?t) replace professional development conferences, retreats, forums, etc. Twitter, or whatever is next that replaces it (that?s inevitable, right?) should be made a part of professional development in schools; it is professional development that can happen every day, even every hour. That being said, many teachers already balk at the idea of incorporating technology they are less than comfortable with, and Twitter is not necessarily the easiest to use. While its interface is fine, the etiquette, the chats, and other minutiae could take some time to figure out how to navigate, especially if the user is reticent in the first place. However, educators will need to figure out how to clear these hurdles, because the conversation is too rich and valuable for any teacher in this century to miss.

Last time, we looked at what we should be eating at the various life stages in Dietary Requirements. We have looked before at the importance of a balanced diet and energy balance. Unfortunately a lot of illness/disease prevalent in Ireland is linked to an unbalanced diet. We?ll have a look first at the most pressing diseases: obesity, diabetes, and coronary heart disease? Why are we what we eat?

Obesity

Obesity occurs when a person is 20% over the correct weight for their height. Take a man whose correct weight is ten stone, at 11 stone he would be overweight, but at 12 stone he would be obese. It?s not exactly something that happens overnight, yet it is one of the biggest medical problems worldwide. More than a quarter of Irish children are now overweight or obese (Irish Times, May 2012), so it?s not just adults. The main cause of obesity is, quite simply, when energy input is greater than energy output.

ScalesCauses of Obesity:

  • Lack of exercise.
  • Sedentary lifestyle/occupation.
  • Calorie intake is greater than calorie output.
  • Increased consumption of convenience and fast foods.
  • Depression or boredom, leading to comfort eating.
  • Hormonal imbalance and some medications such as steroids.

Symptoms and health problems associated with obesity:

  • High cholesterol.
  • Stroke.
  • Heart disease.
  • High blood pressure.
  • Diabetes.
  • Gallstones.
  • Infertility.
  • Difficulties in childbirth/during surgery.
  • Poor self-esteem.
  • Varicose veins.
  • Difficulty breathing.
  • Reduced life expectancy.

Overcoming Obesity and Dietary Guidelines:

  • Follow a varied, balanced, low calorie diet.
  • Build up to exercising most days in the week.
  • Eat three well-balanced meals; snacks, if necessary, should be healthy (e.g. a banana).
  • Aim for the five-a-day of fruit and vegetables.
  • Choose white meat and fish as sources of protein.
  • Grill and steam foods, rather than boiling them.
  • Drink plenty of water, restrict alcohol.
  • Switch to low-fat version of dairy products (e.g. cheese).
  • Avoid refined carbohydrates such as sugar and white bread, instead choose high fibre varieties (e.g. brown bread, Weetabix, etc.).
  • Keep convenience foods to a minimum and eat fresh foods as much as possible.
  • Slow, regular weight loss (rather than a crash diet) will be far more beneficial in the long run, and is more likely to have long-lasting effects.

Diabetes

Diabetes mellitus is caused when the pancreas doesn?t produce any or enough insulin, which is necessary to control the level of sugar in the blood. As a result, glucose does not get used in the muscles and body cells for energy and instead builds up in the blood causing damage to the kidneys and eyes. Diabetes is rapidly becoming a huge problem worldwide.

Types of Diabetes:

  • Type 1 ? (also known as Insulin Dependent Diabetes, juvenile or childhood onset) Deficient insulin production by the pancreas, requires daily administration of insulin by injection.
  • Type 2 ? (also known as Non Insulin Dependent or adult onset) A result of the body?s ineffective use of insulin. This type comprises 90% of people with diabetes and is largely the result of excess body weight and physical inactivity. Previously associated with adults only, but now also occurring in children. It is generally treatable with diet alone or with diet and tablets.

Symptoms of Diabetes include:

  • Frequent urination.
  • Thirst.
  • Constant hunger.
  • Weight loss.
  • Vision changes.
  • Fatigue.

Specific dietary requirements that should be followed in order to manage the condition:

  • Eat three regular meals a day to stabilise blood sugar levels.
  • Reduce sugar intake, use artificial sweetener, and eat fruit with low sugar content.
  • Eat a healthy diet of between three and five servings of fruit and vegetables a day.
  • Follow a low glycaemic index (GI) diet; eat starch-based carbohydrates for slow energy release.
  • Reduce saturated fat intake.
  • Increase fibre foods and starch-based carbohydrate foods.
  • Eliminate refined sugar products from the diet.
  • Increase intake of high fibre foods.
  • Reduce high salt foods.

Coronary Heart Disease (CHD)

CHD is a massive problem in Ireland. In 2006, 12% of all deaths in those under 65 were as a result of CHD. We have the highest rate in this age bracket in the EU. Quite simply, this is not a problem we can afford to ignore.

Atherosclerosis

Atherosclerosis

The coronary arteries have the very important function of carrying blood to and from the heart. When fatty substances, including cholesterol, start to build up on the walls of the arteries they begin to narrow. This hardening of the arteries, where the blood flow is partially blocked, is known as atherosclerosis. This will develop over time, but has very serious consequences, such as angina and heart attacks.

Cholesterol

Cholesterol is a soft, wax-like substance found in every cell. It is mainly made by the liver with some being obtained from animal foods. Cholesterol always seems to get bad press, though we do need some of it as an essential component in cell membranes, in bile, and in some hormones. In the blood, it helps to transport fats. There are two types you should remember from when we looked at Lipids? ?H? for ?Hoover? and ?L? for ?Loser?. Well, I did say at the time, whatever worked for you!

  • Low-density lipoprotein (LDL) ? found in saturated fats, may cause hardening of the arteries.
  • High-density lipoprotein (HDL) ? found in unsaturated fats, may help reduce the hardening effect of cholesterol.

Risk Factors for CHD:

  • Men over 45.
  • Women over 55.
  • Heredity.
  • High cholesterol and high blood pressure.
  • Obesity and a poor diet.
  • Little exercise.
  • Stress.
  • Smoking.
  • Excess alcohol consumption.

Lifestyle changes to prevent CHD:

  • Maintain a healthy weight.
  • Do not smoke.
  • Avoid excess alcohol.
  • Exercise, as this lowers LDL and raises HDL.
  • Reduce stress

Dietary guidelines for preventing/treating CHD:

  • Reduce intake of saturated animal fats.
  • Avoid foods with high hidden fat content.
  • Choose low-fat dairy products.
  • Reduce the intake of salt in the diet.
  • Use mono and polyunsaturated oils.
  • Increase fibre intake, reduce refined carbohydrate intake.
  • Eat lean meat or fish, rather than red meat, as a source of protein.
  • Eat plenty of fruit and vegetables.
  • Grill, bake, steam, or boil food.
  • Avoid creamy sauces.
  • Watch energy balance, remember that energy input needs to equal energy output.
